This Legal Use Disclosure is intended to inform clients of the specific responsibilities, limitation, and considerations when requesting DNA testing services for legal purpose through Hub City DNA.

Legal Admissibility
Hub City DNA offers DNA testing services that are suitable for use in legal proceedings such as child custody, immigration, paternity disputes and other matters requiring genetic evidence. All legal testing services follow strict chain-of-custody procedures as mandated by the AABB-accredited laboratories we work with.

Chain of Custody Requirements
To maintain legal admissibility, all parties involved must present valid government-issued identification and provide witnessed signatures at the time of sample collection. Any deviation from this process may result in the test being deemed inadmissible by the requesting authority.

Limitations of Legal Impact
Hub City DNA is not responsible for the interpretation or outcome of any legal matter in which the test results are submitted. The results provided reflect only the outcome of the DNA analysis and are not legal determinations or advice. Clients are advised to consult with an attorney for legal guidance.

Release of Results
DNA test results for legal purposes will only be released to individuals as authorized at the time of service and as required by the requesting legal entity or court order. All results are handled in accordance with HIPPAA and applicable privacy regulations.

Client Acknowledgement
By choosing to proceed with a legal DNA test through Hub City DNA, you acknowledge that you understand the chain-of-custody requirements and that the outcome of the test does not guarantee a particular legal decision or result.

Hub City DNA Refund Policy
Non-Refundable Fees
A $75 Booking Fee is required at the time of scheduling and is non-refundable, regardless of the appointment outcome. In addition, the following fees are also non-refundable:

* Same-Day Appointment Fee
* Mileage or Travel Fees
* Lab Processing or Lab Analysis Fees
Hub City DNA Terms of Service
Agreement Overview
This Service Agreement is made between the client (referred to as "Client')) and Hub City DNA (referred to as 'Provider"). By scheduling and/or receiving services, the Client agrees to abide by the terms set forth in this document.

Services Provided
Hub City DNA provides mobile DNA collection services and submits collected samples to accredited laboratories for analysis. Services may include paternity testing, immigration DNA testing, relationship testing, and other forms of genetic analysis.

Client Responsibility
*Provide accurate and truthful information for identity verification.
*Arrive on time to scheduled appointments or provide adequate notice for rescheduling.
* Understand and comply with the consent process and any documentation requirements.
*Pay all fees as agreed upon at the time of booking, including an non-refundable charges.

Fees and Payments
A $75 non-refundable Booking Fee is due at the time of scheduling. Additional fees, such as mileage, lab processing, and same-day service charges may apply and are also non-refundable. All payments must be completed before test results are released.

Cancellation and Refunds
Clients may cancel or reschedule appointments with at least 48 hours notice. Cancellations made within 24-48 hours may incur a cancellation fee. No-show or same-day cancellations will result in forfeiture of the full service fee. Refunds are not provided for services already rendered or for any non-refundable fees.

Confidentiality
Hub City DNA adheres to strict confidentiality and privacy practices. Client information and test results will only be shared with individuals or agencies authorized by the client, in accordance with HIPAA and applicable state laws.
​
Limitation of Liability
Hub City DNA acts solely as a sample collection provider and is not responsible for laboratory testing errors, delays in results, or legal interpretations of the results. The client agrees to release Hub City DNA and its representatives from any liability related to the outcome or use of the DNA test results.
