Non-Refundable Fees:  $75 Booking Fee, Same Day Appointment Fees, Mileage Fees and any Lab Processing Fees are Not Refundable.


Rescheduling: Original $75 Booking Fee will apply to the first rescheduled appointment if notice is given at least 24 hours in advance. Appointments can be rescheduled once without penalty
if notice is given at least 24 hours in advance.
